Attendance Forms
Please fill out the following Google Forms for the respective absence or appeal! For an excused absence, you must have it submitted by 11:59 pm on the day of the HOSA meeting after the one you missed. This applies for ALL absence forms! (See WCTA HOSA 101 slides for more information)

Missed a meeting for academic, personal, or unrelated reasons? Fill out this form to excuse your absence from a general meeting!
